The VVS service was introduced to our practice about a year ago as a cardiology
second opinion service but quickly expanded to other disciplines.

For me and all the vets in the practice, this was the very first contact to the much-talked-about telemedicine.

We were pleasantly surprised to find that VVS provided all the necessary kit to start the service
immediately and the support to use the tools has been extremely good from start.

The veterinary specialists are extremely helpful and patient to explain clearly the different
conditions and they always take extra time to improve my knowledge and, in case of the cardiology
service, my ultrasound scanning skills.

Communication is definitely one of the strongest points. The VVS specialists help us achieve a
diagnosis quicker, formulate a treatment plan and pass the information on to the clients in a clear way.

🐾 Meet Abbie the adorable patient having her specialist live-guided cardiology work up with VVS and the wonderful team at Medivet Waltham Abbey! 😍 

During her VVS live-guided cardiology work-up Abbie has a full echo performed by her case vet, guided by the VVS cardiology specialist. An ECG was also recorded and assessed. 

Following the work-up, a full written specialist report was provided, and continued case support for the veterinary team was offered. VVS also provides CPD certificates for case vets to recognise their time spent working with the specialist. 

We are thrilled to be working with Medivet Waltham Abbey and to continue to collaborate with them so they can offer specialist support for their patients in their clinic.

This week has been an exciting one for us at Virtual Veterinary Specialists!

We kicked off the week by installing the VVS workstation with the amazing team at @Moleseyvets. We are thrilled to continue providing our live-guided specialist cardiac work-ups and more to the vets and patients of Molesey Vets.

On Wednesday evening, we had the pleasure of assisting with an event at @Pet_People_Vets for veterinary colleagues delivered by the always brilliant Rob Foale. The talk focused on imported dogs and infectious diseases, and it was a real hit. We're always excited to learn and share knowledge with our peers in the profession.

🥞 Pancake Recipe 🥞

Makes 6 large pancakes:

100g SR flour
Pinch salt
Pinch caster sugar
1 free-range egg and 1 yolk
Half a pint of milk of choice
2 tbsp melted butter

1. Add all dry ingredients to a bowl
2. Combine all wet ingredients in a separate jug
3. Add wet ingredients into the dry ingredients slowly, whisking as you go
4. Heat a non-stick pan on medium heat with a little butter and add a ladle of batter at a time, flip the pancake once cooked underneath. Cook until brown on both sides. 
5. Enjoy with a topping of your choice!

According to a study in 2015 by Niessen et.al. what is the overall prevalence  of acromegaly in diabetic cats? 

  A) 24.8%
 B) 8.4%
 C) 82.8% 
  D) 12.7%

Answer.....

Acromegaly used to be thought as an unusual condition in cats but it is now known to cause diabetes mellitus and be much more common than we first thought.

The 2015 study by Niessen et.al showed a 24.8% (answer: A) prevalence of acromegaly in diabetic cats. This study also demonstrated that the majority of acromegalic cats do not exhibit the classic signs of acromegaly such as enlargement of extremities (broad facial features and club paws etc) and organomegaly. Only 24% of the attending clinicians suspected that any of the positive cats had acromegaly. 

To screen for acromegaly in newly diagnosed diabetic cats, measure serum IGF-1.

A study by Quimby & Lunn (2013) showed that oral mirtazapine improved appetite, reduced vomiting and increased weight gain in cats with naturally occurring CKD. The evidence for transdermal mirtazapine is also good, with a study showing mirtazapine-treated cats gained 3.9 ±5.4% body weight over 14 days which was significantly greater than placebo (0.4 ±3.3% weight gain). In comparison, whilst maropitant significantly reduced the frequency of vomiting in one study (Quimby et al. 2015), it had no effect on appetite score or body weight. Finally, omeprazole did not affect body weight or other subjective assessments of food consumption in a small study by McLeland et al. (2014).

🍗 Are we starving our patients for too long before anaesthesia? 🍖 

How do you talk to your clients about pre-anaesthetic starvation in your patients? 

 “Don’t give him any breakfast” 🥐 

 “Starve from 10pm the night before” 🕙 

 “Don’t give her anything to eat after midnight” 🌃 

This often leads to very prolonged starvation times, especially if owners give the animal their evening meal at the usual time and then nothing but water afterwards. For a patient fed their supper at 6pm, that has an anaesthetic at 11am the next morning, they have been starved for 17 hours! 

The consensus is that the likelihood of regurgitation is greater with starvation times of 12-18 hours, than it is with shorter starvation times of 2-4 hours. In people, it was found extending the duration of preoperative fasting was associated with increased gastric acidity and an increased occurrence of reflux. 

Current American Animal Hospital Association guidelines are for a fasting period of just 4-6 hours in healthy adult patients with no water withhold. Animals less than 2kg in weight or less than 8weeks of age should be fasted for 1-2 hours only. The only patients that should have a more prolonged starvation are those at risk of regurgitation e.g. those with delayed gastric emptying. 

The way we communicate with our owners should also change; try talking about a “give a final small meal” time rather than a “starve from” time.
